2005 – 2006 Consent & Injury Release Form

I, , am the parent or legal guardian of who is a student at Naperville School District 203. I give permission for the aboved named student to participate in any and all NNHS Cheerleading activities held during the 2005-2006 season. As ‘s legal guardian, I hereby release, indemnify and hold harmless Naperville Community School District 203 for any claim of damage arising out of personal injury, including but not limited to aggravation of a prior injury which my child may suffer during his/her participation in any NNHS Cheerleading Team sponsored event, regardless of its cause, except to the extent that the injury was the direct and proximate cause of Naperville Community School District 203’s willful and wanton conduct.
