New 2005 State Law for Oral Health Examination |

As of
July 1, 2005 a new state law went into effect. All students in
Kindergarten, 2nd and 6th grades will be required to have an oral health
examination performed after May 15, 2005 and before May 15th, 2006.
Please
have your dentist fill out District 203's form after your child's exam
and return it to the Health Office no later that May 15th, 2006. Dental
forms are available in the Health Office or by
clicking here.
If you
need a referral to a dentist or financial assistance to obtain your
child's exam, please contact the Health Office for assistance from your
school nurse
