Listed above is the fourth grade supply list for the 2005-2006 school year. Please make sure that your child has these on the first day of school. Elmwood Home & School is again selling these supplies in a pre-paid packet. The price of the packet is $42.00, which is competitive with or lower than local merchants. Since you are pre-paying, your child will receive the packet on the first day of school from his/her teacher. No shopping required!!
The school store will be open from 8:15-9:15 a.m. the first two days of school and each Thursday from 8:15-9:15 a.m. throughout the school year. Special items are available to purchase as well as all of the supplies included in the packet.
To reserve a school supply packet for your child, please fill out and return the lower portion of this form and include a check for $42.00 made payable to Elmwood Home & School. There will not be extra packets available if you do not order in advance. Orders are due Friday, April 15, 2004! You may combine orders for all your children at Elmwood on one check, but please submit an order form for each packet.
