New Summer School Employees

Please complete all of the following forms and return them in person to the Summer School Director who has hired you.

On the Medical form, you must have proof of a negative TB test completed within the last 12 months and a Statement of Good Health from your physician from a physical completed within the last 12 months.

On the 1-9 U.S. Department of Justice Employee Eligibility form, the preferred forms of identification are:

Driver's License and Social Security Card
or
Driver's License and original State or County issued Birth Certificate
or
Passport

If you are a teacher, you must provide a copy the front and back of your current Illinois certificate.

If you are an assistant, you must provide a copy of your Aide Approval certificate. If you do not have Aide Approval, you may go to the following web-site:

to print the forms. Please take the forms and your transcripts to the Regional Office of Education in Wheaton. Assistants who will work in special education classes are exempt from this requirement.
