Dear Substitute Teacher/Tutor:
Thank you for your interest in substitute teaching and/or tutoring in Naperville Community Unit School District 203. The following materials are necessary to complete your file for the substitute teacher/tutor list:
The following are required documents which must be submitted to the Human Resources Office:
- A copy (front and back) of your teaching or substitute certificate registered in DuPage County for the current school year..
- Two forms of identification in conformance with federal immigration laws (driver's license with Social Security card, or state issued birth certificate, or a passport. Originals must be brought in to the Human Resource Office with application.
- A copy of your college transcripts
The following are links to the forms to be completed and a link to District 203 schools along with a map . Please print the forms, fill them out, and return to the Human Resource Office with the above required documents. Note: You must have Acrobat Reader to view the following forms. - Completed Application Form.
- Completed Medical Form which includes proof of freedom of tuberculosis dated within the year.
- A signed Criminal Background Investigation Form.
- A signed Child Abuse Form.
- Completed Teachers' Retirement System Form.
- Completed Federal W-4 Forms.
- Completed Illinois W-4 Forms.
- Completed Direct Deposit Form.
- Completed Employment Eligibility Verification.
- Completed Internet Access Form.
- Completed Substitute Profile Form.

Please bring in all forms and documents to Human Resources as soon as they are complete. When you have done this, you will be assigned to a meeting.
